Bonus points if Band Name - Album Name - Song Name are all on the same recording.

For example Black Sabbath debut 'Black Sabbath' also has a song called Black Sabbath.

Led Zeppelin’s song Houses of the Holy isn’t on their album Houses of the Holy. It’s on Physical Graffiti, which is basically b-sides and, IMHO, better.
